Observed no allergy disclosure visible in the facility. CA: The permit holder shall notify consumers by written notification of the presence of major food allergens as an ingredient in unpackaged food items that are served or sold to the consumer. PIC will create and post in public view an allergy disclosure statement.
12A - contamination prevented during food preparation, storage, display

Observed fish submerged in a container of water in the walk in cooler. CA: Except as specified in paragraphs 3 and 4 of this subsection, unpackaged food may not be stored in direct contact with undrained ice. PIC will remove fish from water.
15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used
Regulation: 511-6-1.05(1)(i) - nonfood-contact surfaces (c)
Observed cardboard used as a surface to line shelves in dry storage room. CA: Nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material. PIC will remove cardboard from shelves.

Observed fume hood with an accumulation of grease and debris. CA: Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. PIC will have fume hood cleaned.
17C - physical facilities installed, maintained, and clean

Observed motorized scooter stored in dry storage room. CA: The premises shall be free of items that are unnecessary to the operation or maintenance of the establishment, such as litter or equipment that is nonfunctional or no longer used and litter. PIC will remove vehicle from dry storage room.
