Observed leak at faucet of three compartment sink. CA: A plumbing system shall be repaired according to law; P and maintained in good repair. PIC will have faucet repaired.

Observed roach activity as evidenced by a live roach found in the women's restroom. CA: The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by: routinely inspecting incoming shipments of food and supplies; routinely inspecting the premises for evidence of pests; using methods, if pests are found, such as trapping devices or other means of pest control as specified under subsections (6)(e), (6)(m), and (6)(n) of this Rule; and eliminating harborage conditions. PIC will contact a pest control company.

Observed ceiling tiles around the facility with an accumulation of dust. CA: The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. Except for cleaning that is necessary due to a spill or other accident, cleaning shall be done during periods when the least amount of food is exposed, such as after closing. PIC will have the ceiling tiles cleaned.

Observed the coving by the vegetable prep table in need of repair. CA: In food service establishments in which cleaning methods other than water flushing are used for cleaning floors, the floor and wall junctures shall be coved and closed to no larger than one thirty-second inch (1 mm). PIC will have the coving repaired.

Observed a TCS food item such as chicken nuggets in hot holding not being maintained at 135 degrees Fahrenheit or above. CA: Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below or 135°F (57°C) or above. PIC placed the chicken nuggets in the fryer to reheat.

Observed open packs of cheddar cheese and ham slices being stored in the walk-in cooler. CA: Except as specified under paragraph 2. of this subsection and when cooling as specified in DPH Rule 511-6-1-.04(6)(e)2.(ii), storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination. PIC wrapped the pack of cheese with saran wrap.

Observed no sanitizer at the three-compartment sink. CA: A chemical sanitizer used in a sanitizing solution for a manual or mechanical operation at contact times specified under subsection (8)(b)3 of this Rule shall meet the requirements specified in DPH Rule 511-6-1-.07(6)(g), shall be used in accordance with the Environmental Protection Agency (EPA)-registered label use instructions. PIC will provide the three compartment sink with a sanitizer that meets DPH requirements and used in in accordance with EPA label instructions.

Observed the interior of the ice machine not clean to sight or touch. CA:Equipment food-contact surfaces and utensils shall be cleaned at a frequency specified by the manufacturer; or absent manufacturer specifications, at a frequency necessary to preclude accumulation of soil or mold. PIC will have the interior of the ice machine washed and sanitized.

Observed broken floor tiles in the drive thru area. CA: Except as specified under subsection (2)(d) of this Rule and except for antislip floor coverings or applications that may be used for safety reasons, floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. PIC will have the broken floor tiles repaired.
