High Priority - Employee touched face/hair and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Manager touching face and hair several times with bare hands then continued to handle clean containers and equipment without washing hands between tasks.

High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. chili (46F,46F - Cold Holding) in bottom of standing reach in cooler across from register at front counter with multiple bags of other food stored in top of cooler appearing to block air flow, per manager door has been consistently open for about an hour, items moved to other reach in cooler. Corrective Action Taken

High Priority - Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. meatballs (121F - Hot Holding); Au jus (125F - Hot Holding); cooked onions (126F - Hot Holding) in steam well across from register at front counter, per manager all items where heated up and placed in well about 1 hour ago, manager reheated all items in microwave to over 165F and placed in different steam well. Corrected On-Site

Basic - Floor soiled/has accumulation of debris. Under large ice machine and standing reach in coolers/freezers and triple sink in prep area have a build up of mold like substance and food debris.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Exterior of hand wash station sink across from front counter has a build up of lime scale and mold like substance around handles. Around soda nozzles of soda machine at drink station in dining area has a build up of white substance. Door handles of reach in cooler closest to triple sink in back prep area has a build up of food debris. Interior of cabinets at drink station in dining area have a build up of liquid debris.

Basic - Reach-in cooler interior/shelves have accumulation of soil residues. Back top wall of several standing reach in coolers have a build up of brown substance.

Basic - Single-service articles stored on a soiled surface. Bags of to go straws and other single use items stored in cabinets under drink station in dining that has a build up of debris inside cabinets.

High Priority - Live, small flying insects found. Three live flying insects near mop sink.

High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. On mop sink with added splitter, vacuum breaker missing from non-chemical side of splitter.

Basic - Interior of oven/microwave/conveyor oven/toaster has accumulation of black substance/grease/food debris. Interior of toaster conveyor oven has build up of food debris.

Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Int deflector plate of large ice machine in prep area.

Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Employee drink stored over single service items on rack in back storage area of kitchen.
