Observed mildew on the inside of the ice bin of the ice machine in the kitchen area, In equipment such as ice bins and beverage dispensing nozzles and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment:
(I) At a frequency specified by the manufacturer; The manager will have the inside of the ice machine cleaned and sanitized by 2/25/26
12B - personal cleanliness
Regulation: 511-6-1.03(5)(j) - hair restraints (c)
Observed a employee prepping drinks in the kitchen area with a beard longer than 1/2 in length, Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch. The manager will order beard guards by 3/15/26
12C - wiping cloths: properly used and stored
Regulation: 511-6-1.04(4)(m) - wiping cloths, use limitation (c)
Observed wet wiping cloths being stored on the stainless-steel table between usage,Containers of chemical sanitizing solutions in which wet wiping cloths are held between uses shall be stored off the floor and used in a manner that prevents contamination of food, equipment, utensils, linens, single-service, or single-use articles. COS the cleaning towel placed in the container between usage correctly.

Observed the prep top cooler not cold holding properly to keep the PHF item under 41-degree Fahrenheit, Utensils shall be maintained in a state of repair or condition that complies with the requirements specified under subsections (1) and (2) of this Rule or shall be discarded. The manager will have maintenance come check the prep top cooler for temperature problem by 2/19/26
