1 - adequate, approved, permitted, protected potable water supply; no cross connection or back siphonage

well that supplies all rooms with water has not been tested for years. it is located in a messy shed behind building. unknown how close septic systems are to it
6 - premises free of pests and rodents; pesticide applied in accordance with laws, rules, and label

observed cockroaches under sink in room 4. tenets were storing dirty dishes under sink and have been for some time. more then 10 flies found in room 1 and fly sticky tape had over 100 flies on it
8 - annual gas fired equipment inspection on file; gas units equipped with automatic safety and/or 100% cut-off pilot
Regulation: 511-6-2-.15(5) - heating and fire safety-inspection
Property owner had fire extinguishers checked by a private company, but could not produce a fire safety report. Must get a inspection from the county fire station official
27 - all furniture, waste receptacles, draperies, carpets; clean, and in good repair, considered the property of and furnished by the tourist accommodation

Rooms 1, 2, 4, 5and 11, all had furniture brought in, such as couches, sofas, used tires and old ovens. correct by getting rid of all furnishing that is not a permanent part of the room for rent

Observed no posted Tourist Accommodation posted in public view. The permit is not posted in a manner that customers can easily and readily view. Post permit in public view
16 - toilet/lavatory/bathing facility fixtures and rooms clean and maintained; properly lighted, clearly marked, proper signage, and all in good repair

Observed mold, water damage and disrepair in bathrooms in rooms 1, 4, 5, 6, 7, 9. correct by repairing floor boards, cleaning walls and filling holes
17 - proper ventilation; no sign of mold, mildew, odors or moisture

No bath room ventilation. mold observed in showers in rooms 4, and 9
25 - floors, walls, ceilings, windows, doors clean, and in good repair; appropriate for location and operation

signs of water damage in every room inspected. walls are dirty and have many holes. correct by replacing water damaged ceiling and wood and repainting. lots of peeling paint in all bathrooms, needs scraped off and repainted
38 - no pets, guests and/or unauthorized personnel in kitchen/food prep area; employees smoking/eating in unapproved location

room one had a puppy, room two had cats, room 4 had cat food in frig but no other signs of animals, room five had a 20 gal fish aquarium and rooms 1, 4, 5, 6, and 11 all smelled of cigarettes'

Exterior plumbing loose and not securely attached. This is for sink drain near septic tank in back of building. room 11 hand sink hot water leaks and wont turn off without turning the shut off valve
35 - electrical work and materials in compliance; grounded and weatherproof outlets, properly located power lines
Observed ungrounded modified outlet outside with no cover. outlet is not rated to be out side
