Facility unable to provide documentation for a CFSM. Edgar, a kitchen staff member is reportedly the CFSM, but did not have a certificate or any proof.

Observed an unidentified spray bottle in the front food service area. Observed multiple spray bottles of sanitizer hung in contact with food items. Observed chemical bottles and one box of chemical stored in contact with single use items and a bottle of honey. Toxic items must be stored separately from all food items and single use items.

Observed single use items stored by the handwash sink below the paper towels in a location to be easily contaminated by staff hand washing. Observed single use to-go containers stored on the floor of the storage room. Observed baked goods boxes on a table in the dining area with unrestricted patron access. All single use items must be stored in a way that prevents their contamination.
