Observed dead black bug under the bed of room 12 and on the floor of room 34. CA: Appropriate measures shall be taken to limit the presence of dead insects

Observed holes in the comforter in room 6 and stains on the sheets of room 12. CA: Establishment shall provide clean linen in each room

Observed the walls in multiple rooms inspected were soiled. CA: Floors, walls and ceilings shall be properly maintained in good repair

Observed spray bottles with clear liquid in them with no label, inside of the laundry room. Per conversation with staff, the chemical is "air freshener". COS: Employee inside of laundry room labeled the bottle on the cart and the bottle the housekeeping woman walked into the laundry room with during the conversation. CA: Any spray bottle used for cleaning tourist room shall be labeled with contents

Observed dead roach inside of refrigerator and in toilet of room 23. Observed two dead insects smashed into sheets in room 26. Observed dead bugs behind night stand in front of dresser in room 7

Observed facilities annual gas fire equipment inspection is past due. Owner states that inspector agreed to come this week and continues to reschedule. Informed owner that he will need to get another inspector to check equipment if the current one is unavailable to complete inspection

Throughout multiple rooms I observed broken toilets, a bent shower frame, stained and damaged tubs, rusted and damaged microwaves

Observed all rooms had excessive stains and/or burn holes in the linen. When observing the laundry room, I observed the clean, white linen was being used only for adjoining hotel (Studio 6) and the visibly stained linen labeled "Budget/Stay Over" and "Dog Linen" is being used for Budget

Observed mesh on both laundry room doors has been damaged. Doors also would not close completely unless forced and then they would pop back open. Wood on the double door that leads to adjoining hotel has shifted allowing an additional entrance for pest and/or rodents

Observed headboard in room 23 had been removed and deep gouging of paint not fixed. Observed base boards and floors around base boards in all rooms had excessive visible dirt and debris on them

Observed ice machines had a build up of a green substance inside of the dispensing portion. CA: Ice machine must be cleaned prior to continued use

Room 5: Observed freezer had deep gauges in it. Inspector opened the refrigerator and observed a large block of ice hanging from the top of the refrigerator portion. CA: All furniture and equipment in guest rooms must be maintained in good repair and clean

Observed dust, cobwebs and debris on the floors of all rooms inspected. Spoke with hotel manager about ensuring house keeping staff is also cleaning the floors so that they are clean to sight and touch

Observed central toilet is not available to patrons as the front office has been locked to the guest. Patrons and guest are required to call over to the adjoining hotel.

511-6-2-.08(1) - Toilet Facilities-Provided and Accessible Toilet, lavatory, and bathing facilities shall be provided at all tourist accommodations except as provided in paragraphs (13) and (16) of this Rule. Such facilities shall be easily accessible, convenient and available to patrons at all times

Observed dead bugs on the floor in room 4. Observed flies present in rooms throughout the facility. Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. The premises shall be kept in such condition as to prevent the attraction, harborage or feeding of insects or rodents

Observed microwave in room 6 with excessive rust on the interior of the door. All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation must be maintained in good repair, clean, and free of vermin

Observed a sex toy between the mattress and box spring in room 4. Observed dirt, stains, and holes present in the sheets on beds in all inspected rooms. Observed excessive stains on mattress cover in room 24. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin

Observed facility's most recent inspection report covered up by another piece of paper and sitting to the side of the office away from the guest window. The most recent tourist accommodation inspection report form shall be prominently displayed in public view at all times, between five feet and seven feet from the floor and in a public area such as the registration desk, where it can be read at a distance of one foot away

Observed outside dumpster missing a lid altogether ad missing a drain plug to prevent leakage. All outside refuse or garbage storage containers shall be constructed of durable metal or other approved types of materials, which do not leak and do not absorb liquids and shall be provided with tight-fitting lids or covers and shall be kept covered when stored

Observed all doors to rooms inspected were not tight fitting. Observed laundry room single door left open. Observed laundry room double doors closed but with a large gap all the way around the door. Openings to the outside shall be effectively protected against the entrance of rodents and insects by tight-fitting doors, closed windows, screening, controlled air currents or other means

Observed dust and debris on floors throughout the facility. Observed yellow-orange liquid-like substance on the decorative ceiling in room 24. Floors, walls, ceilings, windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean

Observed dirt buildup on food contact surface of dispensing ice machine. Scoops, ice buckets, and other ice handling equipment, shall be of easily cleanable material and construction and shall be kept clean

Restricted use pesticides, as specified in 40 CFR 152 Subpart I - Classification of Pesticides shall be applied only by a licensed professional. All pesticide applications must be in accordance with current state and federal laws and the product label. Applying pesticides within buildings using area fog dispersal methods or as warranted under DPH Rule-.20(4)(c) is restricted to licensed pest control professionals as part of an integrated pest management program

Observed hair and debris between comforter and sheets. Also observed multiple burn holes and stains on all of the sheets and bed covers. Observed box spring in rooms 15 and 25 had duct tape over the large holes. Observed duct tape holding plastic covering over the bed in room 25. Observed brown, black and brownish red stains on the sheets and bed covers in the rooms

Observed that the hotels most recent inspection report is not posted.

511-6-2-.05(7) - Inspection Posting Any tourist accommodation inspection report addendum(s), completed by the Health Authority to document observations, violations, and corrective actions resulting from an inspection need not be displayed, but must be made available by the tourist accommodation operator to the public upon request

Observed facility does not have tourist accommodation permit posted.

11-6-2-.04(1)a - Permit Display Any person operating a tourist accommodation shall obtain and display a valid DPH tourist accommodation permit issued by the Health Authority

Observed hot water nozzle in room 25 was broken. Observed light fixture in the bathroom of room 15 was broken so the light was left uncovered with the bulb exposed

Observed rooms 5, 6, 7, and 25 did not have proper ventilation in the bathrooms. Toilet lavatory and bathing facilities shall be mechanically ventilated. Where ventilation ducts are used, ducts from toilet rooms shall not be connected into return ventilation ducts to any other room. All new or major renovation of a tourist accommodation will require mechanical ventilation to be installed to the outside of the building separate from heating and air-condition systems. Owners did renovations and the ventilation was removed. Consecutive violation

Observed all doors to rooms inspected were not tight fitting. Observed laundry room doors were left open. Observed cat laying inside laundry room with food and water being provided to cat in front of shelf where linen is held. Explained that the doors must remain closed at all times and alternate means such as a mesh screen door to prevent the entry or harborage of insects or rodents. Explained that a risk control plan meeting will need to be held due to this violation being observed 3 consecutive times

Observed the walls in room 15 and 25 had been painted dark green. CA: Walls and ceilings shall be easily cleanable and light colored

Observed rooms 5, 6, 7, and 25 did not have proper ventilation in the bathrooms. Toilet lavatory and bathing facilities shall be mechanically ventilated. Where ventilation ducts are used, ducts from toilet rooms shall not be connected into return ventilation ducts to any other room. All new or major renovation of a tourist accommodation will require mechanical ventilation to be installed to the outside of the building separate from heating and air-condition systems

Observed rooms 15 and 25 did not have anti-slip surface on bath tub floor. Both tubs were visibly dirty and paint is peeling off of the bottom of the tub. CA: Each bathing facility shall be kept clean and in good repair

Observed live and dead bugs in room 7, 8, 9, and 10.

511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly

Observed tub and counter top visibly not kept clean or in good repair. Corner of glass mirror in bathroom area was broken. Tub was visibly stained and dirty.

511-6-2-.08(4) - Toilet Facilities-Cleaning and Maintenance Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair. While being used by tourists, every surface of a bathtub, shower, shower enclosure, toilet and lavatory, which may come in contact with a person’s body, must be sanitized each day, unless the guest has declined regular tourist room services. If a tourist declines regular tourist room services, the tourist accommodation facility must ensure that these surfaces are cleaned and sanitized at least once per week and between tourists

Observed 7-10 did not have proper ventilation in the bathrooms. Toilet lavatory and bathing facilities shall be mechanically ventilated. Where ventilation ducts are used, ducts from toilet rooms shall not be connected into return ventilation ducts to any other room. Where ventilation ducts are used, ducts from toilet rooms shall not be connected into return ventilation ducts to any other room. In existing tourist accommodations permitted prior to the adoption of this rule, the current ventilation must prevent odor, mold, mildew, and moisture. All new or major renovation of a tourist accommodation will require mechanical ventilation to be installed to the outside of the building separate from heating and air-condition systems

511-6-2-.13(3) - Insect and Rodent Control-Openings Openings to the outside shall be effectively protected against the entrance of rodents and insects by tight-fitting doors, closed windows, screening, controlled air currents or other means. Screen doors shall be self-closing and screens for windows, doors, skylights, transoms and other openings to the outside shall be tight-fitting and free of breaks. Screening materials shall not be less than sixteen mesh to the inch. Screen doors for sliding patio doors will not be required to be self closing except in food service, preparation and utensil washing areas of permitted food service establishment kitchens and bed and breakfast inns

Observed room 32 is not being kept in sound construction. Observed multiple sections where the base boards were missing. Wall outlet where A/C unit is plugged in is broken and hanging from the wall. Dirt, debris and water marks could visibly be seen on the walls of the room. The lamp shades were tattered and had an excessive amount of dust and debris under the lamp shade. The water heater inside of the room is exposed to guest.

511-6-2-.14(1) - Floors, Walls, and Ceilings Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean. In all new constructed and extensively remodeled residential kitchens used in bed and breakfast inns, coved base molding may be required. Walls and ceilings in residential kitchens shall be easily cleanable and light colored

Observed 7-10 did not have proper ventilation in the bathrooms. Toilet lavatory and bathing facilities shall be mechanically ventilated. Where ventilation ducts are used, ducts from toilet rooms shall not be connected into return ventilation ducts to any other room. Observed rooms 7-10 did not have anti-slip surface on bath tub floor. All tubs shall anti-slip surface in each of the bath tubs.

511-6-2-.08(10) - Anti-Slip Surface Anti-slip tubs slip strips, appliqués, or slip-proof mats shall be provided in each bathing facility and shall be kept clean and in good repair

Observed two bottles of liquid being stored in bottles labeled Air Freshener. One of the liquids were blue and the other was yellow. Upon conversation with PIC I was informed neither was air freshener. COS: The PIC took the bottles and placed the correct liquid in each.

511-6-2-.17(2) - Housekeeping Carts Housekeeper carts shall be so arranged that clean replacement supplies, clean linens, and cleaned and sanitized multi-use equipment and utensils shall be protected from soiled items being removed from each room. Soiled linens and refuse shall be placed in appropriate containers on housekeepers’ carts. Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents. Cleaners and sanitizer shall be used according to the manufacturer’s use direction and label
