Observed live roach bugs in room 106 and laundry room. CA: Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. COS: Roach bugs were removed from areas. Discussed adding weather stripping to doors to prevent bugs from entering rooms and eliminating cardboard boxes and items unnecessary to facility
27 - all furniture, waste receptacles, draperies, carpets; clean, and in good repair, considered the property of and furnished by the tourist accommodation

Observed interior of microwave in room #: 104, 106, 132, and 131 soiled with dried food debris. CA: Condition of all furniture, appliances, and other accessories, in the tourist accommodation must be maintained in good repair, and clean. COS: Housekeeping notified and cleaned interior of microwaves

28 - beds, mattresses, mattress pads, springs, frames, pillows, and bed coverings clean; vermin free and in good condition; bed linen properly supplied, changed and laundered

Observed bed sheet with hair and comforter with cigarette burn in room 106. Observed mattress stained in room 132. Observed comforter with cigarette burn in room 131. CA: Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. COS: House keeping cleaned areas mentioned above and changed comforters
24 - openings to outside and physical facilities protected by tight-fitting doors, screening, or other means
Regulation: 511-6-2-.13(3) - insect and rodent control-openings
Observed doors in rooms # 104, 106, 132, 131, and 203 without tight fitting doors. CA: Openings to the outside shall be effectively protected against the entrance of rodents and insects by tight-fitting doors, closed windows, screening, controlled air currents or other means. No COS can be observed. Inspector must return to verify weather stripping has been installed on doors
25 - floors, walls, ceilings, windows, doors clean, and in good repair; appropriate for location and operation
Regulation: 511-6-2-.14(1) - floors, walls, and ceilings
Observed holes by shower area/curtain rod in room 104, Observed towel bar in disrepair causing surface to be not smooth and cleanable in room 132. CA: Floors, walls, ceilings, windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean. No COS can be observed, inspector must return to verify areas have been repaired
19 - anti-slip tubs/showers, slip strips, mats or appliqué products provided and in good condition
Regulation: 511-6-2-.08(10) - anti-slip surface
Observed bathtub in room 104 without anti-slip surface. CA: Anti-slip tubs slip strips, or slip-proof mats shall be provided in each bathing facility and shall be kept clean and in good repair. No COS can be observed, inspector must return for follow-up to verify anti-slip surface has been installed
