Observed multiple broken/missing floor tiles throughout kitchen. Water ponding in areas where floor tiles are damaged. Floors need to be regrouted to prevent water and food debris from collecting between tiles. Corrective Action: work order placed for tiles and grout. Will perform informal follow-up on 3/31/25 to verify tiles and grouting have been redone. Performed informal follow-up on 3/31/25. Grouting and tiling was not redone. Will follow-up with store and district manager in a couple weeks.

Observed improper cold holding of multiple tcs food items above 41 degrees in walk-in cooler without active time control. See temperature charts. Per discussion with PIC food items were within time parameters. Corrective Action: PIC was notified, walk-in cooler door isn't being closed properly causing temperatures to rise above 41F. Discussed ensuring walk-in cooler door is properly closed at all times. After walk-in cooler door was properly closed ambient temperature was now reading 39F.

Observed improper cold holding of tcs food items above 41 degrees in reach in cooler without active time control. See temperature chart. Corrective Action: PIC was notified, per discussion with PIC food items were within time parameters. Reach-in cooler is not able to maintain temperature of 41F or below and current ice bath was melted ice water. Discussed increasing frequency at which ice bath is replaced or temporary time as a public health control until cooler is repaired and able to maintain temperature of 41F or below.

Corrective Action: PIC was notified, roll of paper towels was provided next to handwashing sink. Discussed availability of paper towels at handwashing sinks all times.
