Observed multiple pans stored under pass-through and several lids in dry storage with accumulated debris. CA: Equipment food-contact surfaces and utensils shall be clean to sight and touch. COS: PIC moved dirty pans to three compartment sink.

Observed grime build-up on interior walls, handle, and baffle of ice machine. CA: Equipment food-contact surfaces and utensils shall be clean to sight and touch. No COS can be observed. Inspector must return within by 4/16/2026 to recheck.

Observed pan of container of rice held over 24 hours with no date marking. CA: Refrigerated, ready-to-eat, time/temperature control for safety food prepared and held in a food establishment for more than 24 hours shall be clearly marked to indicate the date or day by which the food shall be consumed on the premises, sold, or discarded, when held at a temperature of 41°F (5°C) or below for a maximum of 7 days. COS: PIC discarded container of rice.

Observed no allergen poster at service counter and drive-thru. CA: The permit holder shall notify consumers by written notification of the presence of major food allergens as an ingredient in unpackaged food items that are served or sold to the consumer. COS: No COS can be observed. Inspector must return to recheck by 4/16/2026.
11C - approved thawing methods used
Regulation: 511-6-1.04(6)(c) - thawing (c)
Observed mac and cheese and gravy with meat thawing in main kitchen in standing water. CA: Time/temperature control for safety food shall be thawed according to approved methods, such as: thawing under refrigeration, under cold, running water, in a microwave oven, or other approved methods. COS: PIC moved items back to walk-in freezer.
15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used
Regulation: 511-6-1.05(6)(a) - good repair & proper adjustment (c)
Observed non-functional cooler, fryer, and rethermalizer in main kitchen. CA: Equipment shall be maintained in a state of repair. COS: no COS can be observed. Inspector must return by 04/16/2026 to verify that cooler, fryer, and rethermalizer were repaired or work order in place. Date of repair must be verified. Once repaired, proper function of cooler, fryer, and rethermalizer must be verified before use.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed fried chicken breading crumbs on to-go boxes in dry storage. CA: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. COS: employee assigned to clean to-go boxes.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed green buildup on wall in main kitchen. Observed floors and shelving throughout facility with heavy food debris/grease buildup. CA: The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. No COS can be observed. Inspector must return by 04/16/2026 to ensure facility has been cleaned.
