6-1A - proper cold holding temperatures
Regulation: 511-6-1.04(6)(f) - time/temperature control for safety; cold holding (p)
Observed multiple TCS foods (Sliced Tomato 50 degrees Fahrenheit, Slaw 46 degrees Fahrenheit, Ranch Dressing 56 degrees Fahrenheit) cold holding at greater than 41 degrees Fahrenheit in a prep top cooler on the main service line. CA: TCS foods must be cold held at 41 degrees Fahrenheit or less. COS: TCS food that had been stored in the cooler for 4 hours or greater were discarded (Sliced Tomato, Slaw). TCS foods that had been stored in the cooler for less than 2 hours were moved to the walk in cooler to be rapidly cooled (Ranch Dressing).

Observed TCS food (Raw Chicken 52 degrees Fahrenheit) cold holding above 41 degrees Fahrenheit in the reach on cooler on the main cook line. CA: TCS foods must be cold held at 41 degrees Fahrenheit or less. COS: Per the CFSM, the chicken had been placed into the cooler less than 1 hour prior, the chicken was moved to the walk in cooler in order to be rapidly cooled.
15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used
Regulation: 511-6-1.05(6)(a) - good repair & proper adjustment (c)
Observed prep top cooler on main service line not properly functioning and with a broken lid. Observed reach in cooler next to fryer not properly functioning. Observed raw chicken breading station with a missing lid and the reach in area not functioning. CA: Equipment must be maintained in a state of good repair. COS: No COS noted at this time. The facility will have the coolers serviced by a licensed technician. This will be rechecked by the Health Department.
17A - toilet facilities: properly constructed, supplied, cleaned
Regulation: 511-6-1.07(2)(l) - toilet rooms, enclosed (c)
Observed self-closing mechanism on the women's restroom broken. CA: A restroom shall be completely enclosed and provided with a tight-fitting and self-closing door. COS: No COS noted at this time. The facility will have the self-closing mechanism repaired. This will be rechecked by the Health Department.
17B - garbage/refuse properly disposed; facilities maintained
Regulation: 511-6-1.06(5)(o),(p) - using drain plugs; maintaining refuse areas (c)
Observed damaged drain plug on dumpster. CA: All dumpsters and waste handling units must have drain plugs in place. COS: No COS noted at this time. The facility will have the drain plug replaced. This will be rechecked by the Health Department.
