Regulation: 511-6-1.03(2)(o) - person-in-charge duties (pf)
Observed missing employee health policy agreements for current employees at facility. CA: Food employees and conditional employees must be informed in a verifiable manner of their responsibility to report to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food. COS: PIC had employees on shift sign health policy agreement signifying they understand the employee health policy. Health dept will return to recheck once all employees have had a opportunity to sign agreement.
15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used
Regulation: 511-6-1.05(6)(a) - good repair & proper adjustment (c)
Observed ice condensation on ceiling of walk-in freezer and icicles dripping onto food boxes. CA: Equipment shall be maintained in a state of good repair. COS: Door to walk-in freezer was blocked and unable to close properly. Boxes were moved to allow door to close. Food boxes were relocated away from icicle dripping and food box contaminated by drippage was discarded. Recommend walk-in freezer to be serviced/repaired. Health dept will return to recheck.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(4)(b) - designated areas for employee activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (c)
Observed personal food stored on top shelf in cooler with customer foods at biscuit making station. CA: Areas designated for employees to eat and drink shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination. COS: PIC removed personal food from cooler.
