6-1A - proper cold holding temperatures
Regulation: 511-6-1.04(6)(f) - time/temperature control for safety; cold holding (p)
Observed cold TCS food stored above 41 F (see above) in the front display cooler and in the prep-top cooler closest to the cash register. COS. To be discarded 4 hours after it was removed from the walk-in. Code: TCS food that uses cold holding as a control for safety shall be held at 41 F or below.
4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(7)(a)1 - equipment, food-contact surfaces,& utensils (pf)
Observed accumulation of mold/mildew-like substance on the inside of the soda nozzles. Code: Food contact surfaces shall be clean to sight and touch.
8-2B - toxic substances properly identified, stored, used
Regulation: 511-6-1.07(6)(c) - storage, separation (p)
Observed that the facility is storing chemical spray bottles over their 3-compartment sink and clean utensils. COS. Removed. Code: Chemicals shall stored in a manner that prevents contamination of food and food contact surfaces.
12B - personal cleanliness
Regulation: 511-6-1.03(5)(g) - jewelry (c)
Observed employee wearing bracelet while preparing and handling food. COS. Removed. Code: With the exception of a plain band ring, no jewelry may be worn on the hands or wrists while preparing/handling food.
12C - wiping cloths: properly used and stored
Regulation: 511-6-1.04(4)(m) - wiping cloths, use limitation (c)
Observed wet wiping cloths stored on the counter in the front prep area. COS. Wet wiping cloths placed into sanitizer solution. Code: Wet wiping cloths shall be stored in sanitizer solution at the appropriate concentration between uses.
16B - plumbing installed; proper backflow devices
Regulation: 511-6-1.06(2)(r) - system maintained in good repair (p, c)
Observed that the hand sink in the back has water leaking from the base of the faucet, the spray hose has a continuous leak, and water drips from the 3-compartment sink faucet. Code: Plumbing system and fixtures shall be maintained in a state of good repair.
13A - posted: permit/inspection/choking poster/handwashing
Regulation: 511-6-1.02(1)(d) - displaying of the inspection report (c)
Observed that the facility does not have their most recent inspection report posted. Code: The most recent inspection report shall be posted within 15 feet of the front door, 5-7 feet from the floor, readable from a distance of 1 foot away, and in a conspicuous place.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed heavy accumulation of debris and standing water on floor of walk-in cooler, minor accumulation of debris on the floor of the walk-in freezer, accumulation under the shelves of the dish drying rack, and accumulation of standing water next to the standing freezer near the rear exit; observed damaged and missing ceiling tiles near the restrooms. Code: Facility shall be thoroughly cleaned and cleaning frequency shall be enough to prevent accumulation; facility shall be maintained in a state of good repair.
