CA: Food shall be stored in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination. Observed a food container of mushrooms in the reach-in cooler and two food containers of dessert in the deep chest freezer without any lids or coverings. COS: PIC provided plastic coverings for each food container.

CA: Food shall be safe, unadulterated, and honestly presented. Observed two dead flying insects in a food container of cheese in the reach-in cooler. COS: Food employee discarded the food container of cheese and placed container in the dish area.
Regulation: 511-6-1.03(6) - clean-up of vomiting and diarrheal events(pf)
CA: A food establishment shall have an EPA registered disinfectant effective against Norovirus available on-site at the food service establishment when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food service establishment. Observed their chlorine cleaning solution of "Purox Klean" that is used for cleaning clothes. Observed no EPA number and sodium hypochlorite at 3%.
Regulation: 511-6-1.04(6)(g) - ready-to-eat time/temperature control for safety food, date marking (pf)
CA: Refrigerated, ready-to-eat, time/temperature control for safety food prepared and held in a food establishment for more than 24 hours shall be clearly marked to indicate the date or day by which the food shall be consumed on the premises, sold, or discarded, when held at a temperature of 41°F (5°C) or below for a maximum of 7 days. The day of preparation shall be counted as Day 1. Pf Upon arrival observed food employee discarding food containers of tomatoes and beef from prep top cooler and reach-in cooler. Observed dates missing on food containers of lasagna that food employee stated was cooked the previous day. COS: Food employee wrote the date the food item was prepared on the food container with a black sharpie.

CA: Cooling shall be accomplished in accordance with the time and temperature criteria specified under DPH Rule 511-6-1-.04(6)(d) by placing the food in shallow pans; separating the food into smaller or thinner portions; using rapid cooling equipment; stirring the food in a container placed in an ice water bath; using containers that facilitate heat transfer; adding ice as an ingredient; other effective methods. Pf Observed a food container of salsa cooling in an ice bath smaller than the container on the counter next to the prep top cooler. COS: Food employees moved food containers to a bigger ice bath.

CA: A food service establishment shall be protected against the entry of insects and rodents by filling or closing holes and other gaps along floors, walls, and ceilings; closed, tight-fitting windows; and solid, self-closing, tight-fitting doors. Observed the door in main lobby partially open and small insects flying around in the kitchen. COS: PIC closed the door in the lobby.
