Observed 2025 Food Service Permit posted in public view. Advised PIC that a current Food Service permit shall be posted in public view. Advised PIC to obtain the 2026 Food Service permit and post in public view.
15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used
Regulation: 511-6-1.05(6)(a) - good repair & proper adjustment (c)
Observed RIC #1 at the front food service with an ambient temperature of 43 degrees F. Advised PIC equipment shall be maintained in a state of repair and condition that meets the requirements specified under subsections (1) and (2) of this Rule. Advised to have unit serviced.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed nonfood contact surfaces of the interior of the front-food service chest freezer with residue and debris. Advised PIC that nonfood contact surfaces shall be kept free of an accumulation of dust, dirt, food residue, and other debris. Advised to clean interior of chest freezer.
17C - physical facilities installed, maintained, and clean

Observed severe food splatter near the front food service half-wall near reach in cooler #1. Advised PIC The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. Advised PIC to thoroughly clean half-wall near the front-food service reach in cooler #1.
