Observed food employee picked up single service lid from floor and proceeded to handle bag of fries from reach-in-freezer. Food employees shall clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles and: after touching bare human body parts other than clean hands and clean, exposed arms. COS: Had food employee cease prep, wash hands, and don gloves.

Observed food contact surface of interior ice machine with stains in need of cleaning. Equipment food-contact surfaces and utensils shall be cleaned: at a frequency specified by the manufacturer; or absent manufacturer specifications, at a frequency necessary to preclude accumulation of soil or mold.

Observed no food allergen disclosure posted for the public regarding menu food items. The permit holder shall notify consumers by written notification of the presence of major food allergens as an ingredient in unpackaged food items that are served or sold to the consumer.

Observed dish machine in disrepair at time of inspection. Equipment shall be maintained in a state of repair.
