Observed chair with rip in the back in room 103. Observed microwave with pink substance and debris in the following rooms: 206 (pink substance), 207 (debris). Observed debris in the coffee maker in the following rooms: 206, 207, and 103. Advised PIC all furniture, appliances, must be maintained in good repair, clean, and free of vermin
28 - beds, mattresses, mattress pads, springs, frames, pillows, and bed coverings clean; vermin free and in good condition; bed linen properly supplied, changed and laundered

Observed stains on linen in the following rooms: 102 (bed skirt), 103( bed sheets), 106( bed sheets), 206 (blanket). Advised PIC beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin
16 - toilet/lavatory/bathing facility fixtures and rooms clean and maintained; properly lighted, clearly marked, proper signage, and all in good repair

Observed shower curtains with black substance in the following rooms: 103 and 207. Also observed stains on the ceiling in the following rooms: 206, and 106. Advised PIC walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair. Also advised toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair
