Observed in the main kitchen, four containers of diced grilled chicken in a reach-in unit that was unplugged. Food had been prepared on 10/7.
CA: PIC discarded items. During preparation and cooling, time/temperature control for safety food shall be maintained at 41°F.

Observed mildew-like material on gaskets and area around gaskets in the main kitchen.
RCA: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

Observed facility to be actively using dish machine while the dish machine was not dispensing sanitizer.
RCA: PIC needs to call ECOLAB so that they can fix the dish machine, in the mean time facility will use 3-compartment sink to sanitize dishes. Warewashing Machines, Automatic Dispensing of Detergents and Sanitizers. A warewashing machine that is installed shall be designed and equipped to: automatically dispense detergents and sanitizers.

Observed soda machine to have a black build up.

During the inspection, the chemical concentration in the warewashing machine was found to be undetectable, yet employees were using it despite being aware of the problem. COS: PIC set up 3-comp sink and will not use dishwash machine until it is maintenanced.

At the time of inspection, there was a rag and a container in the main front hand sink. CA/COS: PIC removed items and will ensure employees hand sinks for handwashing only.
