At time of inspection, observed a small gap of daylight visible underneath the back door. Seal gap to prevent pest entry (add weatherstripping, etc.).

Regulation: 511-6-1.06(2)(o) - using a handwashing sink- operation & maintenance (pf)
At time of inspection, observed boxes stacked in front of the front handwashing sink, blocking access to the sink. COS: PIC moved the boxes, providing access to the handwashing sink for use. Rule: "A handwashing sink shall be maintained so that it is accessible at all times for employee use."
Regulation: 511-6-1.06(5)(f) - outside receptacles, design & construction (c)
At time of inspection, observed one of the dumpster lids open. COS: PIC instructed employee to closed the dumpster lid. PIC states she will remind employees to check routinely, to ensure that the dumpster lids are closed.
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
At time of inspection, observed ceiling tiles missing in the back storage area. Also observed the A/C duct is sagging through the open ceiling tile grid. PIC states she has put in a work order to repair/replace the A/C ducting, and then the ceiling tiles will be replaced. At time of inspection, also observed accumulation of debris under/behind/around shelving and equipment. Clean often enough to prevent accumulation.
