Observed CFSM posted in this facility also listed as the CFSM in another location (Polk County, listed Paul Chang exp. 11/1/27). CA: Certified Food Safety Managers must be designated to one food service establishment only and maintain and renew certification in accordance with the requirements of the examination taken.
4-2A - food stored covered
Regulation: 511-6-1.04(4)(c)1(iv) - packaged & unpackaged food, food stored covered(c)
Observed multiple food items stored uncovered in reach-in-freezer and RIC. CA: Unpackaged foods should be stored covered to prevent potential contamination, with the exception of active cooling. COS: PIC placed lids and covered food containers.
11A - proper cooling methods used: adequate equipment for temperature control
Regulation: 511-6-1.04(6)(e) - cooling methods (pf, c)
Observed PIC unaware of cooling food items in RIC. Raw chicken being cooled was covered with lid in RIC. CA: Proper cooling methods include active monitoring and knowledge of cooling parameters. Food being cooled should not be tightly covered during active cooling parameters. COS: Per conversation with kitchen staff, raw chicken had been prepped 3 hours prior to inspection, and was uncovered to allow rapid cooling to take place. Discussed proper cooling methods and cooling parameters with PIC.
12A - contamination prevented during food preparation, storage, display
Regulation: 511-6-1.04(4)(q) - food storage (c)
Observed boxes of raw chicken wings stored directly on walk-in-freezer floor. CA: Except as specified in paragraphs 2. and 3. of this subsection, food shall be protected from contamination by storing the food: (i) In a clean, dry location; (ii) Where it is not exposed to splash, dust, or other contamination; and (iii) At least 6 inches (15 cm) above the floor. COS: PIC placed boxes of raw chicken wings on shelving in freezer.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(4)(b) - designated areas for employee activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (c)
Observed improper storage of personal items above prep top cooler, storage shelves and racks. CA: All employee personal items shall be stored in order to prevent potential contamination of facility equipment or food. COS: PIC relocated all items to designated area.
