Observed several TCS food items improperly cold holding above 41 degrees F in RICs, prep top cooler, and ice bath. CA: All TCS foods being cold held must be 41 degrees F or below. COS: PIC discarded whipped cream and one bag of shredded lettuce. PIC placed all other TCS foods on ice to rapidly cool, as it had been out of temp less than 4 hours.
Observed several pans of food stored without lids/covers throughout facility in reach-in coolers and freezers. CA: Except for when being cooled, food should be stored in packages, covered containers, or wrappings to be protected from contamination COS: PIC placed lids on containers
