Basic - Water leaking from pipe and/or faucet/handle, hand wash sink at rear of kitchen.

Basic - Single-service articles improperly stored. Observed happy boxes and sandwich wrappers (in original box), stored on floor in dry storage area.

Basic - Floor soiled/has accumulation of debris. Observed freezer floor with black build-up and trash scattered throughout.

Basic - Opened employee beverage container in a cold holding unit with food to be served to customers. Observed employee drinks inside cooler behind grill area, on top of food to be served to customers. Manager removed drinks and cleaned area properly. Corrected On-Site Repeat Violation

Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. 3 employee drinks on food prep tables on front line and near grill.

Basic - Walk in freezer floor soiled/ disrepair.

Basic - Wall in disrepair near back kitchen exit. Repeat Violation

Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Drive thru window hall, vent / ceiling condensation and black mold like substance.

Basic - Evidence of Employee eating in a food preparation or other restricted area. Employee personal food not properly identified and segregated from food to be served to the public. Observed employee food at expo station.( open box of chicken nuggets with open sauce). Manger removed food and sanitized area.

Basic - Floor tiles missing and/or in disrepair. Floor tile near frozen drink machine toward cook line.

Basic - Wall in disrepair in dry stock area. Paint peeling from wall.
