High Priority - - From initial inspection : High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed 4 flies under the washboards of the warewashing area. Observed 3 flies under the back prep room tables closest to the walk in cooler. Repeat Violation Admin Complaint - From follow-up inspection 2023-11-14: At time of follow up inspection 2 flies observed under the two compartment sink in the prep area. Operator removed container from under the sink and placed out fly trap device under the sink. Admin Complaint Corrective Action Taken

Observed 4 rodent droppings on the under the equipment under the dish machine. Observed 19 rodent droppings under the three compartment sink. Observed 8 rodent droppings by the water heater and oven on the floor. Observed 1 rodent droppings on the water heater. Observed 8 rodent droppings on the floor in the corner of the prep table and walk in cooler. Operator began cleaning, sweeping and removing all observed droppings. Corrective Action Taken Warning - From follow-up inspection 2023-11-14: At time of follow up inspection, 3 rodent droppings observed under the three compartment sink and 1 rodent droppings observed on fly trap under the dish machine. High Priority - Warewashing sanitizing solution exceeding the maximum concentration allowed. Quaternary sanitizer solution in bucket by the smoker turned test strip blue over 500ppm. Operator adjusted the sanitizer to 200ppm. Corrected On-Site
