Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. At front line ice machine; accumulation of black/green mold-like substance in the interior of the ice machine/bin.

Intermediate - Handwash sink not accessible for employee use at all times. Hand wash water turned off due to leak. Operator turned on hand wash sink. Discussed with operator Repeat Violation

Basic - Buildup of food debris/soil residue on equipment door handles.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed in pizza box holding cabinet

Basic - Standing water or very slow draining water in handwash sink, three-compartment sink, or mop sink. Observed at hand wash sink on cook line

High Priority - Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. After 3 attempts Dishwasher (Chlorine 0ppm) - chlorine tub empty with no back up available. Operator will utilize manual sanitizing until chlorine sanitizer is received. Operator contacted technician. Corrective Action Taken
