Basic - Accumulation of debris on exterior of warewashing machine. Observed top of dish machine has food debris and dust on it.

Basic - Cardboard used to line nonfood-contact shelves. Observed soiled cardboard with fryer baskets on them on top of reach in cooler.

Basic - No handwashing sign provided at a hand sink used by food employees. Observed no hand washing sign at the back hand wash sink.

Basic - Bowl or other container with no handle used to dispense food. Observed cup with handle in the cheese on the prep table, employee moved. Corrected On-Site

Basic - Unsealed concrete floor in food preparation, food storage, warewashing area or bathroom. Observed loose concrete floor under a shelf in back of kitchen near the dish area.

Basic - Wet mop not stored in a manner to allow the mop to dry. Corrected On-Site

Basic - Clean equipment and utensils not separated from soiled equipment and utensils at warewashing area. Observed cleaned food containers stored on bottom of wire storage rack beside warewash. Dirty dishes processed and stored on rack above cleaned dishes. Corrective Action Taken

Basic - Cleaned and sanitized equipment or utensils not properly stored. Observed container of cinnamon sugar stored over cleaned dishes at dish storage area.

High Priority - Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed Dishwasher (Chlorine 0ppm), dispenser empty. Manager reloaded sanitizer primed to 50ppm. Corrected On-Site

Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed some black debris on interior drop guard.

Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed loose debris on ceiling on walk in cooler.

Basic - Designated employee eating/drinking/smoking area located in a food preparation or other restricted area causing possible cross contamination. Employee drinking eating area is in kitchen beside ice machine/ prep table and walk-in cooler.

Basic - Employee eating in a food preparation or other restricted area. Observed open beverage glass and twist off drink bottle located in food prep area beside food prep table/ walk-in cooler.

High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Upon arrival Manager had already taken action to cool foods and called maintenance.manager iced the reachin and has added ice bags on top of foods to recool quickly. Observed make table had stopped functioning and foods were not at 41F. Corrective Action Taken
