OBSERVED EMPLOYEE ENTERING FROM BACK DOOR AND GO STRAIGHT TO PREP TABLE TO GRAB VEGETABLES TO PREP PIZZA WITHOUT WASHING THEIR HANDS FIRST. THE FOOD IN THE PREP TABLE IS ALSO USED IN READY TO EAT FOODS (SALADS, SANDWICHES). FOOD EMPLOYEES MUST CLEAN THEIR HANDS AND EXPOSED PORITIONS OF THEIR ARMS IMMEDIATELY BEFORE ENGAGING IN FOOD PREPARATION. COS- EMPLOYEE STOPPED TO WASH HANDS AND INSPECTOR EDUCATED ON WHEN TO WASH.

BACK DOOR DOES NOT FULLY SELF CLOSE AND IS NOT SEALED TO PREVENT ENTRY OF PESTS AND INSECTS. A GAP OF LIGHT CAN BE SEEN. ALL EXTERIOR DOORS MUST BE FULLY SELF CLOSING AND BE PROTECTED AGAINST PEST ENTRY. DOOR MUST BE PROPERLY FIXED AND SEALED WITHIN 30 DAYS.

OBSERVED BUILDUP OF DEBRIS BEHIND AND UNDERNEATH EQUIPMENT THROUGHOUT FACILITY. ALSO OBSERVED HEAVILY STAINED/DIRTY CEILING TILES. ALL SURFACES IN THE ESTABLISHMENT MUST BE CLEANED WITH ENOUGH FREQUENCY TO PREVENT BUILDUP OF GRIME.
