Observed peeling paint on wall near walk in cooler, observed baseboards broken near back door. Observed eroded grout near floor drain in kitchen. Please fix all these physical facilities by 4/12/2025. All physical facilities should be in good repair.

Observed vegetables sink being used as a dumping station. The vegetable sink can only be used for washing vegetables and thawing and cooling ready to eat foods. COS the cooks stop using it as a dump station.
