Observed there is no paperwork on file verifying that there has been a recent gas-fired equipment inspection performed. Informed owner that this inspection must be performed on an annual basis and he said he is going to take care of the issue immediately

Observed black mold around sink and shower seals in rooms 100 and 128. Recommended these rooms be closed until housekeeping has time to do a good deep cleaning of these areas

Observed soiled curtains in room 227. Curtains had stains all over them and just looked unclean. Draperies shall be kept clean and in good repair

Observed mold/mildew in seams of shower in room 101, caulking in need of repair around top of shower in room 101. Observed large hole behind the toilet in room 227. These areas shall be cleaned and repaired as soon as possible. Showers shall be scrubbed and free of mold/mildew at all times. Large hole behind toilet could lead to possible intrusion of pests/rodents

Observed food debris under cushions of chairs in a couple of the rooms inspected. Recommend discussing with housekeeping staff to check all of these areas when cleaning rooms. Furnishings shall be properly maintained, cleaned and kept in good repair

Observed mattress seams tearing in room 205. Mattresses, pads, etc. shall be kept in good repair and clean

Observed inside surface of microwave oven in room 114 was badly chipping away and could contaminate food being prepared in the microwave oven. All furnishings shall be properly maintained and kept in good repair. PIC took a photo of oven and will pass on to management to have oven replaced
Regulation: 511-6-2-.08(4) - toilet facilities-cleaning and maintenance
Observed light bulb cover missing, shower head loose and separated from wall in restroom in room 204. PIC took a photo and noted to let management know this needed replaced. Restroom fixtures shall be maintained in good repair

Observed that there was no anti-slip matt in place in the restroom in room 104. PIC notified housekeeping and housekeeping brought a matt at time of inspection. Anti-slip matts shall be provided and kept clean and in good repair

Observed cleaner in cleaner bottles that were not labeled. All chemicals and chemical containers shall be labeled with it's contents. PIC (Jennifer Hill) stated that "Ecolab sends stickers to place on and label chemical bottles and that she will make sure employees correct this TODAY."
