Observed soiled mattress (badly stained) in room 311. Mattresses shall be kept clean and free of any accumulations of stains, dirt, dust, etc
25 - floors, walls, ceilings, windows, doors clean, and in good repair; appropriate for location and operation

Observed Paint chipping badly on the door frame of the bathroom in room 317. Floors, walls, and ceilings shall be kept clean and well maintained at all times
19 - anti-slip tubs/showers, slip strips, mats or appliqué products provided and in good condition
Regulation: 511-6-2-.08(10) - anti-slip surface
Observed that there was no anti-slip matt available in the shower/tub in room 317. Housekeeping was notified to bring an anti-slip matt to the room at time of inspection
33 - housekeeping carts and storage areas maintained; clean/sanitized items protected from contamination

Observed several spray bottles of sanitizer/cleaning solutions not labeled with common name of chemical in the bottles. All cleaning supply bottles shall be labeled with the common name of the cleaning solution in the bottle/container
35 - electrical work and materials in compliance; grounded and weatherproof outlets, properly located power lines
Regulation: 511-6-2-.18(6) - eletrical supply
Observed that an electrical outlet behind the bed was completely out of the wall and there is a large hole in the wall in outlet area in room 310. Also observed a badly damaged switch/receptacle cover in the bathroom in room 315 that needs replaced. All switch/receptacle covers shall be properly maintained and connected to the wall over the electrical switch/receptacles
