Observed soiled mattress (badly stained) in room 311. Mattresses shall be kept clean and free of any accumulations of stains, dirt, dust, etc

Observed Paint chipping badly on the door frame of the bathroom in room 317. Floors, walls, and ceilings shall be kept clean and well maintained at all times

Observed that there was no anti-slip matt available in the shower/tub in room 317. Housekeeping was notified to bring an anti-slip matt to the room at time of inspection

Observed several spray bottles of sanitizer/cleaning solutions not labeled with common name of chemical in the bottles. All cleaning supply bottles shall be labeled with the common name of the cleaning solution in the bottle/container

Observed that an electrical outlet behind the bed was completely out of the wall and there is a large hole in the wall in outlet area in room 310. Also observed a badly damaged switch/receptacle cover in the bathroom in room 315 that needs replaced. All switch/receptacle covers shall be properly maintained and connected to the wall over the electrical switch/receptacles

Observed mold accumulations in the sink area in room 313 and in the seams of the shower in rooms 406 and 412. These areas shall be cleaned and scrubbed free of mold accumulations by correct-by-date listed above

Observed badly damaged wall next to AC unit in bedroom area of room 318. Floors, walls, and ceilings shall be kept clean and well maintained at all times

Observed ripped curtain in room 303. Observed food crumbs/debris under couch cushions in room 320. Curtains shall be repaired or replaced and sofa cleaned properly by correct-by date. Removal of food debris is essential to prevent intrusion of insects/pests

Observed cigarette holes in pillow and a dead insect inside pillow case in room 405. Pillows shall be cleaned regularly and kept in good condition at all times

Observed holes in the wall in the sink area of the restroom in room 306. Observed mold in the shower in room 405. These areas shall be repaired and cleaned properly by the correct-by date

Observed hole in cabinet in room 418. Observed broken window in room 413. Observed kitchen countertop loose from wall and in need of recaulking in the seams in room 320. These areas shall be repaired by correct-by date and kept in good repair. Window repair is essential to prevent possible injury of guests

Observed bed sheets in room 315 badly stained. Bed linens shall be kept clean. Replace sheets between customers or daily if customer requests

Observed walls and ceilings above shower and door frames going into bathroom area where paint is chipping in room 312. Floors, walls, and ceilings shall be properly maintained and in good repair

Observed smoke alarm cover missing and not in working condition in room 420. Electrical work and smoke alarms shall be maintained in working condition

Observed food debris and trash in the seams of the chair in room 306. PIC notified housekeeping immediately and had housekeeping clean the furniture. All furnishings shall be clean and in good repair

Observed light switch cover and light bulb cover cracked in room 312. Fixtures in restroom areas shall be properly maintained. PIC noted these issues and will notify management of repair needs

Observed large cracks in the wall around the toilet tissue dispenser in room 312. PIC noted this issue and will report to management to have repaired immediately. Walls shall be of good construction, properly maintained and in good repair

Observed no anti-slip matt in room 306. PIC notified house keeping and housekeeping brought an anti-slip matt for this room

Observed mold and mildew around the bottom of the shower(s) in rooms 309 and 417. Toilet rooms and fixtures shall be kept clean and in good repair. Showers shall be cleaned and sanitized daily or if tourist chooses not to have cleaned daily, they shall be cleaned at least once per week and between tourist

Observed hole in the wall around toilet cleanout directly above the toilet in room 420. PIC stated she will have maintenance apply caulk around the hole immediately to prevent entrance of insects or rodents into the room. Walls shall be of good construction, properly maintained and in good repair

Observed no anti-slip matt in shower/tub in rooms 302,309,407,417,420. There shall be anti-slip surfaces or slip-proof matts in each bathing facility and shall be kept clean and in good repair
