Observed no hanging thermometers in walk in cooler and freezer units.
12A - contamination prevented during food preparation, storage, display
Regulation: 511-6-1.04(4)(z) - miscellaneous sources of contamination (c)
Observed dead bug in sugar container, contents dumped.
18 - insects, rodents, and animals not present
Regulation: 511-6-1.07(5)(k) - controlling pests (pf, c)
Observed live flies in kitchen and dead flies on/at windows in dining area. Observed back door not self closing, adjust to close, leaving gap.
16B - plumbing installed; proper backflow devices
Regulation: 511-6-1.06(2)(j) - backflow prevention device when required (p)
Observed mop sink faucet with no vacuum breaker, and hose end stored in sink.
15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used
Regulation: 511-6-1.05(6)(a) - good repair & proper adjustment (c)
Observed crack lid on sugar container and plastic container in use, discarded. Observed counter top with laminate cover peeling off, repair.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed food buildup between fountain heads in lobby.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed grease buildup on walls near fryer units, buildup on piping to canister extinguisher. Observed dust buildup on some air conditioning registers, and water damage to ceiling tile. Observed utility door in poor condition, falling apart towards bottom of door. Reinstall ceiling tile in utility room and repair sheetrock. Repair broken tile at coolers (floor tile).
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(3)(g) - mechanical ventilation, sufficient capacity to prevent buildup of heat, steam, smoke, fumes & odors (c)
Observed men's restroom exhaust fan not working, need to clean women's exhaust fan, vents covered in dust.
