Cold-Holding was assessed and incompliance unless noted in the temperature log. Hot-Holding was assessed and incompliance unless note in the temperature log. If back exit door is prompted open for a delivery or for food employees to take out the trash, ensure a mesh or an air curtain is installed. Fees will be charged accordingly for any violation which cannot be corrected on-site during inspection. Repeat violations may result in permit suspension. 18: .07(2)(m)Observed back exit door prompted open by a brick while food employee was outside taking out the trash. Back exit door does not have an air curtain or a mesh. Outer openings of a food service establishment shall be protected against the entry of insects and rodents by:
- filling or closing holes and other gaps along floors, walls, and ceilings; (C)
- closed, tight-fitting windows; (C) and
- solid self-closing, tight-fitting doors. (C) Corrective Actions: PIC (person in charge) closed the back exit door.

18: .07(5)(l)Observed a roach in the mop sink area. The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by:
- routinely inspection incoming shipments of food and supplies; (C)
- routinely inspecting the premises for evidence of pest; (C)
- having a professional treat the facility; (Pf) and
- eliminating harborage conditions. (C) Corrective Actions: PIC had food employee removed the roach.
