Regulation: 511-6-1.06(2)(o) - using a handwashing sink- operation & maintenance (pf)
Observed 2 hand sinks in food areas (dish room, cook line) blocked by equipment around them, or utensils/dishes in them blocking access. All hand sinks must be kept clear and accessible for handwashing only. COS.
2-2D - adequate handwashing facilities supplied & accessible
Regulation: 511-6-1.07(3)(b) - hand drying provision (pf)
No hand towels or hand drying provisions available at hand wash sinks in kitchen bar, and restrooms, except 1 sink (blocked) had 2 or 3 napkins remaining. Keep all hand sinks stocked with proper paper towels. Staff sent for emergency (truck did not arrive this morning) use paper towel rolls. COS.
6-2 - proper date marking and disposition

Observed numerous date labels with discard date of 3/18/26 (greater then 7 days. Items incorrectly labeled corrected hold date of no more than 7 days with day 1 being date of prep. COS

Observed several live roaches at drink fountain. Control pest by 3/20/26.
14B - utensils, equipment and linens: properly stored, dried, handled
Regulation: 511-6-1.05(10)(e)1,2,4 - equipment, utensil, linens, stored 6" off floor in clean, dry location (c)
Observed utensils with food ends up and exposed, and bowls upright and uncovered. Store items covered and/or inverted.
15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used
Regulation: 511-6-1.05(1)(a) - materials, general requirements (p, c)
Observed rusted shelving by dish area wrapped in plastic liners. Do not use/reuse plastic wrap. Repair or resurface rusted shelving. Correct by 3/20/26.
