-Observed both Quat Sanitizers stored under the Espresso machines in the front service area of the kitchen with chemical concentrations too low (<150ppm). *COS, Sanitizer were refreshed upon request by PIC.

-Observed condensation leak from ceiling in rear kitchen area of the facility near the 3 compartment sink. Catch bucket was collecting drainage. *COS, Have leak repaired and send documentation to Kristen.Tyler@dph.ga.gov within 72hrs.

-Observed leak at top rinse basin of 3 compartment sink in rear kitchen area of the facility. *CTM, Have sink repaired and send documentation to Kristen.Tyler@dph.ga.gov within 72hrs.

-Observed cabinet in front service area of the kitchen under dump sink with accumulations of old liquid food debris. -Observed moldy debris in cabinet under ice bin next to the blenders and strawberry acai tea in the front service area of the kitchen. -Observed fan guards soiled with accumulations of debris in the reach in refrigerator closest to the ice machine in the rear kitchen and the reach in refrigerator in the front service area of the kitchen under the blenders. CTM, Conduct general cleaning and send documentation to Kristen.Tyler@dph.ga.gov within 72hrs.

-Observed clear Quat Sanitizer container stored under Espresso machine with chemical concentrations too low <150ppm. -Observed 2 wet wiping cloths stored on the front service counters of the front service area upon entry into the kitchen. *COS, Sanitizer was refreshed upon request by PIC within proper chemical constraints and wet wiping cloths were stored in sanitizer solution by PIC.

-Observed 1 large red bucket of Quat sanitizer on shelf and 2 smaller clear containers of Quat sanitizer under espresso machines in the front service area of the kitchen with chemical concentrations too low <150ppm. -Observed 3 wet wiping cloths being stored on counter tops in the front service area of the kitchen. *COS, Wiping clothes were stored back in solution by PIC and all sanitizers were discarded and replaced with new solution by PIC upon request. Sanitizers were verified with test strips in proper chemical constraints.

-Observed 3 heavy soiled nozzles (Pike Place, Iced Coffee, and Dark Roast on coffee brewers near swinging door that accesses rear kitchen/ storage area of the kitchen. *COS, PIC empty coffee brewer and started cleaning nozzles (Wash, rinse, sanitize). Will follow up on progress
