Observed deep scoring and gouging on cutting boards. Boards were yellowish in color. CA: Cutting Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they cannot be resurfaced.

Observed dishes on clean rack that had been wet stacked and clean utensils sitting in a bus tub for use that were sitting in water. CA: Pans and utensils must be allowed to dry completely prior to stacking or contact with food items.

Observed fish filet for fish sandwich cooked to 141F. Pack for fish sandwich states that the fish is uncooked a d should be cooked to 165F. Corrected on site by discarding and cooking a new fillet and cooking to 179F.

Observed two hand washing sinks with dispensers that did not appear to work requiring employees to walk around to the hand washing sink by the three compartment sink. Manager replaced one of the dispensers and stated that he has a second dispenser in storage to replace the other dispenser with.
