Observed excessive rust in microwave of room 231. All furniture, draperies, appliances, carpets, and other accessories in the tourist accommodation must be maintained in good repair, clean, and free of vermin

Observed front doors left completely open at time of inspection. Openings to the outside shall be effectively protected against the entrance of rodents and insects by tight-fitting doors, closed windows, screening, controlled air currents or other means

Observed rooms 135, 130, 213, 231, 235 had excessive rust buildup along the ceiling grids in the bathroom. Observed large gaps in the flooring of rooms 213 and 235. Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean

Observed black and pink buildup on the interior of the dispenser on the first-floor ice machine. Ice handling equipment shall be kept clean and in good repair. PIC must clean and sanitize the ice machine

Observed piece of trash paper in nightstand drawer along with 3 plastic forks in room 204. Observed food residue and sprinkles along the edges of a shelf in the refrigerator in room 204. Observed food residue left in microwave in room 204. Observed coffee grounds left in the coffee maker in room 128. Observed water left in the coffee maker in room 204. All furniture, draperies, appliances, carpets, and other accessories in the tourist accommodation must be maintained in good repair, clean, and free of vermin

Observed plastic cup lid in the sheets of the bed in room 204. Observed plastic knife under mattress in room 204. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin

Observed the most recent tourist accommodation inspection report was not posted for public view. The most recent tourist accommodation inspection report form shall be prominently displayed in public view at all times

Observed excessive amount of hair left in bathtub in room 113. Observed the vanity counter pulled off of the wall with a gap between it and the sheetrock of up to approximately one inch in room 128. Observed shower handle leaking in room 128. Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair

Observed all rooms inspected having excessive rust buildup and paint peeling along the ceiling of the bathrooms. Observed pinkish gummy spot on the ceiling of room 209. Observed the sheetrock peeling in the corner of room 113. Observed the molding coming off the base of the wall in room 128. Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean

Observed roach under the sofa bed in room 224. Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. The premises shall be kept in such condition as to prevent the attraction, harborage or feeding of insects or rodents

Observed multiple plastic bags and pieces of trash in the drawer of the nightstand in room 211. All furniture, draperies, appliances, carpets, and other accessories in the tourist accommodation must be maintained in good repair, clean, and free of vermin

Observed excessive hair on the sofa beds in room 137. Observed what appeared to be an elastic wig band in the sofa bed in room 125. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin

Observed multiple side exit doors on the first floor that were not tight fitting with gaps near the edge of the door. Observed the lobby automatic doors turned off and left open at the time of inspection. Observed multiple ceiling tiles missing in the hallway on the first floor. Observed large cracks in the floor of rooms 211, 224, and 233. Observed ceilings in bathrooms of rooms 125, 132, 211, 224, and 233 with excessive rust and buildup on tiles along with peeling paint. Observed molding peeling off in room 233 by the door and next to the air conditioner. Observed gummy bears, baby food, and a baby toy on the floor beneath and next to the sofa bed in room 137. Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean

Observed PIC was unable to provide annual gas fired equipment inspection report. Per conversation with PIC she explained that the inspection was completed but the inspector will not provide report until services have been paid for. CA: The operator shall be able to provide evidence of inspection and/or correction of any deficiency, upon request

Observed most recent inspection report was not posted for public view. CA: The most recent tourist accommodation inspection report form shall be prominently displayed in public view at all times, between five feet and seven feet from the floor and in a public area such as the registration desk, where it can be read at a distance of one foot away. COS: PIC found old inspection report and replaced it with todays report to be posted on the wall

Observed all inspected bathrooms, with the exception of the central restroom, ceilings were rusted with orange discoloration. Observed towel rack in room 235 rusted and peeling. Observed blue and white substance that resembled toothpaste on the faucet of room 236. Observed long brown hair in and on the tub inside of room 224

Observed that all side exits on the first floor where not tight fitting. One of the exits appeared to be not attached to top hinge and was no longer self closing. CA: All doors shall be tight fitting to protect facility from the entrance or rodents and pest

Observed ceiling tile missing on the first floor. Observed large deep crack from the ceiling to the base board in room 223. To the right of the crack in the corner of the wall the paint was bubbled and appeared to have sustained some sort of water damage. Observed deep gauges in the vinyl flooring of room 223. Observed wall socket in room 235 was not covered

Observed multiple large holes in the walls in two separate storage rooms. Observed ceiling tile above storage room on first floor was missing. Observed ceiling tile between rooms 125 and 129 growing a black and gray substance. All wall, floors, ceilings, etc., must be properly maintained.
