Observed cold holding foods in the prep top cooler and reach in cooler unit with temperatures greater than 41 degrees Fahrenheit. Corrected by having food foods thrown out and replaced with new items while sitting on ice bath.

Observed food out of its original container not labeled with its common name. Food and food products removed from its original packaging shall be labeled with its common name.
