Person in Charge (PIC) stated they do not have a current certified food safety manager (CFSM) for the establishment. Corrective Action (CA): PIC instructed all facilities must have a current CFSM on staff with certificate posted in restaurant. REPEAT.
2-2A - management knowledge, responsibilities, reporting
Regulation: 511-6-1.03(2)(o) - person-in-charge duties (pf)
PIC unable to produce signed employee health policy verifying employees have been informed on their duty to report reportable symptoms and illnesses to management. CA: PIC instructed to ensure all employees have reviews and signed the health policy.
2-2D - adequate handwashing facilities supplied & accessible
Regulation: 511-6-1.06(2)(o) - using a handwashing sink- operation & maintenance (pf)
Observed pot stored in kitchen hand sink. CA: PIC instructed to ensure all hand sinks are free and clear and are used for handwashing only. PIC removed pot. COS. REPEAT.
2-2D - adequate handwashing facilities supplied & accessible
Regulation: 511-6-1.07(3)(b) - hand drying provision (pf)
No paper towels stocked at front hand sink. CAL PIC instructed to ensure all hand sinks are stocked with paper towels. REPEAT.
4-2A - food stored covered
Regulation: 511-6-1.04(4)(c)1(iv) - packaged & unpackaged food, food stored covered(c)
Observed multiple items in reach-in cooler stored uncovered. CA: PIC all items not actively cooling are stored covered. PIC covered items. COS.
4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(6)(n) - manual and mechanical warewashing equipment, chemical sanitization-temperature, ph, concentration, hardness (p,pf)
Observed low sanitizer concentration in three compartment sink, below 50ppm. CA: PIC instructed to maintain chlorine sanitizer at 50-100ppm. PIC remade sanitizer. COS.
4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(7)(b) - food contact surfaces and utensils - cleaning frequency (p, c)
Observed mold growth/debris build up on splash plate of ice machine. CA: PIC instructed to ensure ice machine cleaned frequently to prevent mold/debris build up. REPEAT.
14A - in-use utensils: properly stored
Regulation: 511-6-1.04(4)(k) - in-use utensils, between-use storage (c)
Observed scoop with no handles stored in bulk ingredient containers.
14C - single-use/single-service articles: properly stored, used
Regulation: 511-6-1.05(10)(e)1&3 - single-service/single-use items stored 6" off floor in clean, dry location (c)
Observed single service items stored not inverted out of protective liners on top of steam table and boxes of single service items on floor behind front counter. REPEAT.
