6-1A - proper cold holding temperatures
Regulation: 511-6-1.04(6)(f) - time/temperature control for safety; cold holding (p)
Observed multiple food items in the walk-in cooler (mac & cheese, green beans, and rice) cold held above 41F. Manager was advised to discard all items.
6-1B - proper hot holding temperatures
Regulation: 511-6-1.04(6)(f) - time/temperature control for safety; hot holding (p)
Observed grits in the steam well hot held below 135F. Manager reheated grits to proper temperature (one time).
18 - insects, rodents, and animals not present
Regulation: 511-6-1.07(5)(l) - removing dead or trapped birds, insects, rodents, and other pest (c)
Observed dead roaches on the floor of the walk in cooler and underneath the dish machine area.
16B - plumbing installed; proper backflow devices
Regulation: 511-6-1.06(2)(r) - system maintained in good repair (p, c)
Observed reach in freezer with ice build up from the bottom to the middle rack due to a leak in the freezer. Please have reach in freezer leak repaired and ice chiseled.
Observed 3 compartment sink faucet leaking. Have these items repaired and email repair invoice to keundra.golden@dph.gag.ov.
14B - utensils, equipment and linens: properly stored, dried, handled
Regulation: 511-6-1.05(10)(g)2 - kitchen and tableware, properly handled to protect food-/lip-contact surfaces (c)
Observed plates and bowls not inverted above the steam table. Manager placed foil to cover the food contact surfaces of the plates and bowls to prevent potential contamination.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed interior of reach in cooler in front of the grill with build up and rusted shelves. Observed reach in cooler in front of the fryers with debris on the shelves. Observed Observed green shelf in the walk in cooler with mold like substances. Observed build up on the gasket of the reach in freezer by the fryers.
17A - toilet facilities: properly constructed, supplied, cleaned
Regulation: 511-6-1.06(5)(h) - toilet room receptacle, covered (c)
Observed unisex restroom with trash bin missing a lid.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed ceiling tiles in the dining area with brown water spots. Please have ceiling tiles replaces.
Observed grease and water on the floors underneath the equipment. Observed a hole leading to the outside in the wall of the closet by the bar. Manager was advised to purchase steel wool to fill the hole TEMPORARILY until the wall can be professionally repaired.
Observed baseboards in the walk in cooler in bad repair.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(5)(d) - cleaning ventilation system, nuisance & discharge prohibition, cleaned in way not to cause contamination or create a public health hazard (c)
Observed dust accumulation on air vents in the dining area.
