Room 411: Observed desk chair in disrepair and red stain on carpet at the foot of the bed. Room 412 and Room 316: Observed food debris in entertainment center drawer. Room 316: Observed stain on carpet adjacent to trash can. Room 323: Observed interior of microwave with paint peeling, frame of entertainment center in disrepair, debris on carpet underneath pullout mattress, and desk chair in disrepair. Room 216: Observed Formica and desk chair on disrepair. Room 112: Observed light bulb missing in lamp and mold-like accumulation at the back of the refrigerator. Refrigerator must be cleaned and disinfected prior to renting room

Room 411: Observed heavy yellow stain on mattress pads, red stain on sheets,food crumbs and stains on mattress cover, and large yellow stains on mattress. Also observed debris on pullout mattress frame, staining and debris on pullout mattress cover, and multiple yellow stains on mattress. Room 412: Observed hairs on linens, stains on pullout mattress cover, debris on pullout mattress frame, and staining on pullout mattress. Room 316: Observed hair and debris between mattress and box spring. Also observed hair and debris on pullout mattress frame, and hair linens. Room 323: Observed excessive amounts hair between mattress and box spring, and hair on linens. Room 206: Observed yellow fluid and black staining and heavy, brown staining on pullout mattress cover. Room 216: Observed severe yellow stain on mattress cover. Room 112: Observed yellow stains on the mattress cover, mattress cover with hair and tearing, and heavy yellow staining on pullout mattress

Interior of 3rd and 4th floor ice chute with brown/ pink accumulation. Also observed black mold-like accumulation on interior of ice chute on 2nd floor and 1st floor. The 1st floor also contained black mold-like accumulation in drip area of ice machine

Observed employee belongings next clean towels in laundry room. 511-6-2-.17(2) - Housekeeping Carts Housekeeper carts shall be so arranged that clean replacement supplies, clean linens, and cleaned and sanitized multi-use equipment and utensils shall be protected from soiled items being removed from each room. Soiled linens and refuse shall be placed in appropriate containers on housekeepers’ carts. Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents. Cleaners and sanitizer shall be used according to the manufacturer’s use direction and label
