Observed raw shelled eggs stored next to veggie stew in the reach in cooler located in the back room. Raw shelled eggs also stored next to buttermilk in the reach in cooler
511-6-1.04(4)(c)1(i)(ii)(iii)(v)(vi)(vii)(viii) - Packaged & Unpackaged Food Separation, Packaging, and Segregation (P, C) (c) Packaged and Unpackaged Food - Separation, Packaging, and Segregation.
- Food shall be protected from cross contamination by:
(i) Except as specified in paragraph (i)(IV) of this subsection, separating raw animal foods during storage, preparation, holding, and display from:
(I) Raw ready-to-eat food including other raw animal food such as fish for sushi or molluscan shellfish, or other raw ready-to-eat food such as fruits and vegetables, P and
(II) Cooked ready-to-eat food; and P
(III) Fruits and vegetables before they are washed; P
(IV) Frozen, commercially processed and packaged raw animal food may be stored or displayed with or above frozen, commercially processed and packaged, ready-to-eat food.
(ii) Except when combined as ingredients, separating types of raw animal foods from each other such as beef, fish, lamb, pork, and poultry during storage, preparation, holding, and display by:
(I) Using separate equipment for each type, P or
(II) Arranging each type of food in equipment so that cross contamination of one type with another is prevented, P and
(III) Preparing each type of food at different times or in separate areas; P
(iii) Cleaning and sanitizing equipment and utensils;
(v) Cleaning hermetically sealed containers of food of visible soil before opening;
(vi) Protecting food containers that are received packaged together in a case or overwrap from cuts when the case or overwrap is opened;
(vii) Storing damaged, spoiled, or recalled food being held in the food service establishment separate from food, equipment, utensils, linens and single-service and single-use articles; or
(viii) Separating fruits and vegetables, before they are washed from ready-to-eat food.
6-1A - proper cold holding temperatures
Regulation: 511-6-1.04(6)(f) - time/temperature control for safety; cold holding (p)
Observed multiple TCS food items being cold-held in the reach in preparation cooler exceeding 41F. Advised PIC to discard all food items that was not prepped today (6/11/2026). Several prepped items including pancake batter, hashbrowns, and green tomatoes did not meet temperature requirements of 41 degrees or below, As a corrective action PIC will submerge all food pans in ice and use Time as a public health control. All TCS foods on the preparation line being cold held on ice. Time as a public health control will be implemented and documented. After 4 hours the food items should be discarded and the temperatures should be recorded for all food going on ice. PIC stated that a technician has been called and will service the units by 6/12/26. A copy of the service reports must be emailed to sabrina.young@dph.ga.gov and robert.lewis1@dph.ga.gov by 6/12/2026. Ice is a temporary method and cannot be used long term.
511-6-1.04(6)(f) - Time/Temperature Control for Safety; Cold Holding (P) (f) Time/Temperature Control for Safety Food, Hot and Cold Holding. Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below or 135°F (57°C) or above, except that roasts cooked to a temperature and for a time specified in subsection (5)(a)2 of this Rule and reheated using the same temperature and time conditions as cooking may be held at a temperature of 130°F (54°C) or above. 18 - insects, rodents, and animals not present

Observed live German roaches crawling in plastic wrapped foil pans above the microwave. Also observed live German roaches above the handsink on the wall. Dead roaches observed on the floor next to the freezer. Several gnats observed in the storage room. Must have pest control to treat the facility twice this month for German roaches. 511-6-1.07(5)(k) - Controlling Pests (Pf, C) (k) Controlling Pests. The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by:

Routinely inspecting the premises for evidence of pests;

Using methods, if pests are found, such as trapping devices or other means of pest control as specified under subsections (6)(e), (6)(m), and (6)(n) of this Rule; Pf and

Dead or trapped birds, insects, rodents, and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests.

Regulation: 511-6-1.02(1)(d) - displaying of the inspection report (c)
Observed the inspection report not visible to the public. Inspection report was posted on the wall near the entrance but was not visible. PIC advised that the score must be visible at all times.

The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away.

Food service establishments with drive-thru windows will post the current inspection report, and also have the inspection report posted so that a minimum of the top one-third of a copy of the current inspection report is visible through each window allowing customers to easily read the score, date of inspection and establishment information.

At food service establishments with no primary or public door, the current inspection report shall be prominently displayed at all times where the documents can be read by the public from a distance of one foot away. If requirements of this paragraph are not possible because of physical restrictions, a location will be determined as approved by the Health Authority.

The most current inspection report for mobile food service units and extended food service units issued by the local Health Authority having jurisdiction for its inspections shall be prominently displayed in public view during all hours of operation. Such inspection reports shall be prominently displayed at the point of service where the documents can be read by the public from a distance of one foot away.

A food service establishment inspection report addendum need not be displayed, but shall be made available by the food service establishment to the public upon request.
15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used
Regulation: 511-6-1.05(1)(i) - nonfood-contact surfaces (c)
Observed rust build up on the lid of the deep freezer. Sugar lid and container holding pooled eggs was cracked. PIC will discard the damaged equipment.
511-6-1.05(1)(i) - Nonfood-contact Surfaces (C) (i) Nonfood-contact Surfaces. Nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Crate used to store clean dishes contained build up on the side of the crate. Also, observed the interior surface of the steam well with an accumulation. PIC will deep clean by 6/11/26.
511-6-1.05(7)(a)2,3 - Equipment, Food/Nonfood-Contact Surfaces, and Utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (C) 2. The food-contact surfaces of cooking equipment and pans shall be kept free of encrusted grease deposits and other soil accumulations.
- Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
17A - toilet facilities: properly constructed, supplied, cleaned
Regulation: 511-6-1.06(5)(h) - toilet room receptacle, covered (c)
Observed a trash can with a lid not provided for sanitary napkins in the women's restroom.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed the floors, especially underneath equipment, throughout the kitchen in need of cleaning due to accumulation of food debris. Mold observed on the wall and ceiling in the dishwashing area. Old food and debris observed beneath the steps leading to the upstairs storage area. General cleaning and repairs of floors needed in the storage room upstairs.
Observed the ceiling tile missing above the reach in freezer in the kitchen that must be replaced.
Observed the floors and walls damaged in the back of the house near the stairs. Observed the floor tile in bad repair in the stairway area. Also, the floors are not in good repair in the upstairs area. The tile will need to be replaced or properly sealed with cement to make it smooth, easily cleanable, and nonabsorbent. Any holes will need to be properly sealed to prevent rodents, bugs, etc. from entering the facility. The baseboard will need to be properly sealed to the wall in this area too.
