4-1A - food separated and protected

Observed raw ground beef burgers stored above sweet tea and next to cooked pork in the walk in cooler located behind the building.
Also observed walk in coolers not secured behind the building. PIC advised that coolers and storage sheds must be properly locked and protected.
6-1A - proper cold holding temperatures
Regulation: 511-6-1.04(6)(f) - time/temperature control for safety; cold holding (p)
Observed cold TCS foods (tomatoes, lettuce, and cheese) stored above 41 degrees in the preparation top cooler. The reach in cooler was observed at 61 degrees f. PIC will use time as a public health control until the cooler is repaired. PIC will discard products on TPHC within 4 hours.
511-6-1.04(6)(f) - Time/Temperature Control for Safety; Cold Holding (P) (f) Time/Temperature Control for Safety Food, Hot and Cold Holding. Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below or 135°F (57°C) or above, except that roasts cooked to a temperature and for a time specified in subsection (5)(a)2 of this Rule and reheated using the same temperature and time conditions as cooking may be held at a temperature of 130°F (54°C) or above. 6-1C - proper cooling time and temperature

Observed Boston butts were not cooled properly in the walk in cooler. Cooling began at 11:00 am. The temperature was observed at 4:00 pm. PIC advised to discard five Boston butts. Advised PIC to use a cooling log to ensure TCS food items are being cooled properly from 135F to 70F in 2 hours and 70F and below in 4 hours for a total of 6 hours.

General Comments:

Observed employee drinks stored in the food preparation area. Employee drinks contained a top and a lid with a straw. PIC advised to store all employee drinks in a designated area away from food and food contact surfaces.

Employees shall not use any form of tobacco products, or electronic devices that simulate tobacco smoking, while engaged in food preparation or service, nor while in areas used for equipment or utensil washing and storage, food preparation or food storage. Employees shall only use tobacco products or electronic devices that simulate tobacco smoking in approved designated areas.

Employees shall consume food only in approved designated areas separate from food preparation and serving areas, equipment or utensil areas and food storage areas. However, drinking from a single service beverage cup with a secure lid and straw that is handled to prevent contamination of the employee’s hands, the container, exposed food, clean equipment, utensils and linens, unwrapped single-service and single-use articles will be allowed.
18 - insects, rodents, and animals not present
Regulation: 511-6-1.07(5)(k) - controlling pests (pf, c)
Observed several wasp in two storage sheds behind the building. PIC will have pest control to treat both areas. Also observed multiple flies in the kitchen area.
511-6-1.07(5)(k) - Controlling Pests (Pf, C) (k) Controlling Pests. The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by:

Routinely inspecting incoming shipments of food and supplies;

15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used
Regulation: 511-6-1.05(2)(c) - multiuse food-contact surfaces (pf)
Observed the rubber gasket in bad repair on the warming cabinet in the kitchen. Reach in cooler gasket was also observed in bad repair. The gaskets must be repaired or replaced to ensure the food is hot and cold held properly.

- Multiuse food-contact surfaces shall be:

- The requirement in paragraph 1(v) of this subsection does not apply to cooking oil storage tanks, Distribution lines for cooking oils, or beverage syrup lines or tubes.
15C - nonfood-contact surfaces clean

Observed severe mold build up in a white reach in cooler in the shed that is not being used according to the PIC. PIC will have the cooler removed by 6/11/26.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Build up on ceiling, walls, floors, and HVAC vents in the kitchen.

The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.

Except for cleaning that is necessary due to a spill or other accident, cleaning shall be done during periods when the least amount of food is exposed, such as after closing.

If present, playground equipment and associated areas shall be maintained in a clean and sanitary condition. In addition, a cleaning and sanitizing plan along with cleaning and sanitizing records for playground equipment and associated areas shall be maintained within the food service establishment for review upon request by the Health Authority. Further, such cleaning and sanitizing plan for employees to follow in the event of encountering vomitus or fecal matter shall be included and submitted at the time of permit application as specified in DPH Rule 511-6-1-.02(1)(c). Observed the vent in the men's and women's restroom contained heavy rust build up. Vent in the men's restroom not working. PIC will have the vents cleaned and repaired.
Also observed grease build up above the stove in the grease filters. Debris in light shields in the kitchen. Adequate lighting needed in the walk in cooler outside.
511-6-1.05(2)(n) - Exhaust Ventilation Hood Systems, designed to prevent grease or condensation from dripping on food/equipment (C) (n) Exhaust Ventilation Hood Systems. Exhaust ventilation hood systems in food preparation and warewashing areas, including components such as hoods, fans, guards, and ducting shall be designed to prevent grease or condensation from draining or dripping onto food, equipment, utensils, linens, and single-service and single-use articles.
