4-2A - food stored covered
Regulation: 511-6-1.04(4)(c)1(iv) - packaged & unpackaged food, food stored covered(c)
First Consecutive Repeat Violation Observed ice bin lid to be left open upon arrival to facility. Except as specified under paragraph 2. of this subsection and when cooling as specified in DPH Rule 511-6-1-.04(6)(e)2.(ii), storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination. Corrective Actions: PIC immediately closed the ice bin.
12B - personal cleanliness
Regulation: 511-6-1.03(5)(g) - jewelry (c)
Observed employee actively preparing food while wearing a bracelet. Jewelry. Except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food. Corrective Actions: PIC instructed employee to remove bracelet and wash their hands before returning to work.
12C - wiping cloths: properly used and stored
Regulation: 511-6-1.04(4)(m) - wiping cloths, use limitation (c)
Observed wiping cloths not fully submerged in sani buckets with QAC sanitizing solution concentration <150 ppm. Cloths in-use for wiping counters and other equipment surfaces shall be: (i) Held between uses in a chemical sanitizer solution at a concentration specified under DPH Rule 511-6-1-.05(6)(n). Corrective Actions: PIC made new sanitizer solution at proper concentration and ensured employees were aware to fully submerge wiping cloths in sanitizing solution.
14A - in-use utensils: properly stored
Regulation: 511-6-1.04(4)(k) - in-use utensils, between-use storage (c)
Observed 3 sets of metal stirrers to be stored in cupes with water at 71°F, 66°F, and 67°F. Food preparation utensils shall be stored in a container of water if the water is maintained at a temperature of at least 135°F (57°C) and the container is cleaned at a frequency specified under DPH Rule 511-6-1-.05(7)(b)3(vi). Corrective Actions: PIC removed all metal stirrers from cups and placed them in the 3-compartment sink to be washed, rinsed, and sanitized. PIC poured out the cups of water previously used to store stirrers and obtained fresh set of metal stirrers.
17B - garbage/refuse properly disposed; facilities maintained
Regulation: 511-6-1.06(5)(n) - covering receptacles (c)
Observed both sliding metal doors on the sides of the dumpster to be open. Receptacles and waste handling units for refuse, recyclables, and returnables shall be kept covered with tight-fitting lids or doors if kept outside the food service establishment. Corrective Actions: PIC immediately closed the sliding metal doors.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(5)(d) - cleaning ventilation system, nuisance & discharge prohibition, cleaned in way not to cause contamination or create a public health hazard (c)
Observed vent cover in women's restroom to have accumulation of dust and debris. Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination by dust, dirt, and other materials. Corrective Actions: PIC stated vent covers will be cleaned at COB.
