2-2A: .03(2)(m)Observed PIC (Person in Charge) unable to demonstrate in a verifiable manner that food employees were made aware of their reporting responsibilities. Food employees and conditional employees shall be informed in a verifiable manner of their responsibility to report to the person in charge information about their health and activities as they relate to diseases that are transmissible through food. (Pf) Corrective Actions: Employees read and signed employee health reporting agreement.

9-2: .10(5)(c)Observed facility's pizza sauce variance not followed, as container of pizza sauce on the prep counter was not labeled with the time and date it was removed from refrigeration. If granted a variance or a HACCP plan is otherwise required, the permit holder shall:
- Comply with the HACCP plans and procedures that are submitted and deemed in conformance with DPH Rule 511-6-1-.02(6)(a) through (e) as a basis for the modification or waiver; (P) and
- Maintain and provide to the Department, upon request, records specified under DPH Rule 511-6-1-.02(6). (Pf) Corrective Actions: Container of pizza sauce was labeled with the time/date it was removed from refrigeration.

10D: .04(4)(d)Observed multiple working bottles of sauces and seasonings without the common name labeled. SECOND CONSECUTIVE VIOLATION OF THE SAME CODE PROVISION Except for containers holding food that can be readily and unmistakably recognized, such as dry pasta, working containers holding food or food ingredients that are removed from their original packages for use in the food establishment, such as cooking oils, flour, herbs, potato flakes, salt, spices, and sugar shall be clearly and legibly identified, in English, with the common name of the food. (C) Corrective Actions: PIC labeled the containers of sauces and seasonings.

13A: .02(1)(d)Observed most recent inspection report not displayed in public view. The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away. (C) Corrective Actions: Today's inspection report was printed and displayed in public view.

Areas designated for employees to eat, drink, and use tobacco shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination. (C) Corrective Actions: PIC moved the employee items to the designated employee storage area.
