24: .13(3)Observed missing screens in several rooms and common space windows. [.13(3)] Openings to the outside shall be effectively protected against the entrance of rodents and insects by tight-fitting doors, closed windows, screening, controlled air currents or other means.

28: .14(6)Observed multiple beds with hair and debris. [.14(6)] Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly. Corrective Actions: Person in charge instructed housekeeping to remove the hair and debris.

10: .05(1),(7)Facility not displaying the most recent inspection report. [.05(7)] The most recent tourist accommodation inspection report form shall be prominently displayed in public view at all times, between five feet and seven feet from the floor and in a public area such as the registration desk, where it can be read at a distance of one foot away. Corrective Actions: Today's report will be given for posting.

1: .07(2)Hot water temperatures exceeding 120 deg F in guest rooms. Boilers were set to 123 deg F for facility. Ensure boilers are slightly adjusted by 6/13/2025 so hot water does not exceed 120 deg F. Hot water in all guest rooms shall not exceed 120ºF.

6: .13(1)Residential pesticide observed stored on housekeeping cart and in laundry room. Ensure facility only uses pesticide by a licensed pest control operator. Restricted use pesticides, as specified in 40 CFR 152 Subpart I - Classification of Pesticides shall be applied only by a licensed professional. All pesticide applications must be in accordance with current state and federal laws and the product label. Applying pesticides within buildings using area fog dispersal methods or as warranted under DPH Rule-.20(4) (c) is restricted.

6: .13(1)Small insects observed at window sill on rooms 302, and 314. Insect observed in room 314 bathroom. Ensure facility eliminates presence of insects by 6/13/2025. Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. The premises shall be kept in such condition as to prevent the attraction, harborage or feeding of insects or rodents.

24: .13(3)Daylight visible when outer doors to laundry room were fully closed. Daylight also visible in room 314 around the seals of the window. Window was reseated and no light was visible. Ensure daylight is not visible when outer doors to prevent pest entryway by 6/13/2025. 27: .14(5)Coaches in observed rooms with yellowed stains. Ensure coaches have their stained removed by 6/13/2025. [.14(5)] All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin.

28: .14(6)Observed various stains and hair on linens in the following rooms: Room 302- Stains on pull out mattress from sofa and blood-like staining on bed skirts. Room 305- Stained mattress from pull out sofa, hair on pillow cover. Room 314- Hair on pillow case and mattress protector, stained and debris on bed skirt. Room 316- Hair on mattress pad. Ensure clean linens are free from stains and hairs by 6/13/2025. [.14(6)] Washable mattress pads or covers shall be used on all mattresses. 33: .17(2)Working chemical spray bottles and containers observed without any labels of content. Working broom heads, dusters, and other cleaning equipment stored in a manner which may contaminate wrapped single service cups, clean linen, and coffee. Personal employee items stored with and above clean linens and customer foods. Ensure correct labels are added for all working chemical bottles and store personal and cleaning equipment in a manner that does not contaminate clean linens and foods by 6/13/2025. [.17(2)] Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents. Cleaners and sanitizer shall be used according to the manufacturer's use direction and label.
