2-2D: .06(2)(o)Observed ashtrays stored in the handwashing sink. 2ND CONSECUTIVE VIOLATION OF THE SAME CODE PROVISION A handwashing sink shall be maintained so that it is accessible at all times for employee use and not used for purposes other than handwashing. An automatic handwashing facility shall be used in accordance with manufacturer's instructions. (Pf) Corrective Actions: Person in charge moved ashtrays from handwashing sink.

17A: .07(5)(i)Observed the door to the men's bathroom not able to close on its own all the way. Except during cleaning and maintenance operations, toilet room doors shall be kept closed. (C)

17A: .07(3)(e)Observed no toilet tissue in the men's bathroom. A supply of toilet tissue shall be available at each toilet. (C) Corrective Actions: Person in charge added toilet tissue to the men's bathroom area.

Areas designated for employees to eat, drink, and use tobacco shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination. (C) Corrective Actions: Items were relocated appropriately.

4-2B: .05(7)(a)1Observed ice machine with mold-like substance on the interior top part.// Equipment food-contact surfaces and utensils shall be clean to sight and touch. (Pf)

8-2B: .07(6)(c)Multiple butane cans stored above bulk tea and bulk sugar containers in the dry storage area.// Poisonous or toxic materials shall be stored so they cannot contaminate food, equipment, utensils, linens, and single-service and single-use articles. (P) Corrective Actions: Chemicals were relocated.//

12D: .04(4)(g)Observed multiple "washed" avocados and oranges with stickers left on the outside skin surface.// Raw fruits and vegetables shall be thoroughly washed in water, in a sink designated for that purpose only, to remove soil and other contaminants before being cut, combined with other ingredients, cooked, served, or offered for human consumption in ready-to-eat form. (Pf) Corrective Actions: Stickers were removed and produce rewashed.//
