CA: PIC must present documentation to show employees were informed of when to report. No COS can be noted. Observed damaged floor tiles under grill with water pooling. CA: Floors shall be smooth and easily cleanable. No COS can be noted, inspector must return to verify issue has been resolved.

Observed dust accumulation on walk-in cooler fan covers and women's restroom exhaust fan cover. Corrective Action: employee assigned to clean.

CFSM could not provide documentation or otherwise satisfactorily demonstrate during the inspection, that all food employees and conditional employees are informed of their responsibility to report to management information about their health and activities as it relates to diseases that are transmissible through food. Corrective Action: Provided CFSM with employee health policy sheet. Informal follow-up will be performed on Friday, Sept 6 to verify employee health policy documents have been signed by employees. Educated CFSM on 5 symptoms and 6 illnesses associated with food borne illnesses.

Observed cooler pull handles soiled. Corrective Action: CFSM was notified, employee assigned to clean. Will perform informal follow-up on 9/29 to verify.
