Observed dumpster pad area covered with standing water (rain water).
CA: Clean out dumpster pad drain.

Observed equipment in poor repair. Observed large amounts of ice on the floor of the walk in cooler.
CA: Remove all ice from the floor of the unit and have walk in freezer repaired and /or proper adjustments made to correct the accumulated ice on floor.

Observed a few missing wall tiles and coved base tile under hand sink near 4 compartment sink.
CA: Replace missing tiles were needed.
