Observed hand sink in the kitchen next to 3 compartment sink not assessable dripping water and hot water handle not working. Owner stated he turned it off. Observed also hand sink in the kitchen next to cook top not accessible and blocked with a trash can. 511-6-1.06(2)(o) - Using a Handwashing Sink- Operation & maintenance: A handwashing sink shall be maintained so that it is accessible at all times for employee use. Instructed Owner to make hands sink assessable all the time and in good repair for employees use.
4-2A - food stored covered

10D - food properly labeled; original container

Observed squeeze food items in storage containers were not identified with the common name of the food item. Observed several squeeze bottles and food containers in coolers without a label to identify contents. 511-6-1.04(4)(d) - Food Storage Containers Identified with Common Name of Food (C) (d) Food Storage Containers, Identified with Common Name of Food. Except for containers holding food that can be readily and unmistakably recognized, such as dry pasta, working containers holding food or food ingredients that are removed from their original packages for use in the food establishment, such as cooking oils, flour, herbs, potato flakes, salt, spices, and sugar shall be clearly and legibly identified, in English, with the common name of the food. PIC will ensure food that is not stored in its original container is labeled to identify the contents stored.

Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch.

This does not apply to employees such as counter staff who only serve beverages and wrapped or packaged foods, hostesses, and wait staff if they present a minimal risk of contaminating exposed food, clean utensils and linens and unwrapped single-service and single-use articles. PIC advised to usehair restraint while cooking.
16B - plumbing installed; proper backflow devices

Observed hand washing sink next the three compartment sink and pipe under 3 compartment sink leaking. Observed hot water handle not working. (a) Good Repair. All physical facilities shall be maintained in good repair. Owner will fix or replace within 10 days.
15C - nonfood-contact surfaces clean

Observed debris buildup under the dishwasher and three compartment sink.Also the hood vents had oil debris buildup. Observed grease and residues on kitchen equipment, walls and corner of the floor. 511-6-1.05(7)(d) - Nonfood-Contact Surfaces (C) (d) Nonfood-Contact Surfaces. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. PIC advised to do a deep cleaning.

Observed cracks on storage room frame and floor tiles.
