Inspector began washing hands at the hand sink near the grill line and observed hand sink near grill line missing piping (to drain contents of sink to floor drain). Water drained directly onto floor. Corrective Action: A handwashing sink shall be maintained so that it is accessible at all times for employee use. PIC will have hand sink maintained within 48hours. Local Health Authority will verify the sink has been maintained.

Inspector observed visible light through gap at bottom of back door and around the drive thru window. Inspector observed broken self-closer at the back door. Corrective Action: Outer openings of a food service establishment shall be protected against the entry of insects and rodents by Closed, tight-fitting windows and Solid, self-closing, tight-fitting doors.

Inspector observed 5 of 7 light bulbs in the exhaust hoods not working. Inspector observed broken cutting board, steamer with broken leg, shelving with broken wheel, walk in cooler with flooring loosely attached to the unit. Corrective Action: Equipment shall be maintained in a state of repair.

Inspector observed dust accumulation/debris around air vents throughout the facility. Inspector observed a multitude of various holes in the walls throughout the facility. Inspector observed numerous tile-coving missing and loosely attached throughout the facility. Inspector observed floor tiles missing near floor drains. Inspector observed pieces of carpet missing in dining room. Inspector observed front countertop with a broken corner. Inspector observed ceiling missing in the drive through window - insulation exposed. Corrective Action: All physical facilities shall be maintained in good repair. The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. PIC will have drive thru window ceiling maintained within 48 hours. Local Health Authority will verify the ceiling has been maintained. PIC will not serve food through the drive thru window until ceiling is maintained and verified.

Observed a food handler preparing food with a ripped glove. COS- gloves were removed and a clean set of gloves were provided.

No Certified Food Manager for establishment. Advised GM to obtain CFSM as soon as possible.

Observed both water filters heavily soiled. Advised PIC to replace water filters and replace water filters with a frequency that is recommended by the manufacturer-every 3 mos.

Observed no sanitzer in the 3-compartment sink. The sink was drained and refilled reading 300ppm QAC. Advised PIC to ensure the solution is flowing when filling the sinks.

Observed cloths used for table wiping without sanitizer. Advised PIC to use a spray bottle with sanitization solution and paper towels to clean dining tables.

Observed dust accumulation/debris around air vents. Observed various holes in the walls in facility and floor tiles missing near floor drains. Advise more cleaning/more frequent cleaning of floors, walls, and ceilings.

Observed visible light through gap at bottom of back door. Advised PIC to fix the door so it is tight-fitting and no visible light can be seen.

Observed dumpster door left open. Advised PIC to keep all dumpster lids and doors closed to deny pests any access.
