Regulation: 511-6-1.03(3)(c) - certification documentation (pf)
Inspector did not observe CFSM certificate displayed in the facility. PIC states that a CFSM certificate was not available on site. Corrective Action: The original CFSM certificate shall be posted in public view in each food service establishment. An additional copy shall be retained on file at the food service establishment at all times, and shall be made available for inspection by the Health Authority.
4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(6)(n) - manual and mechanical warewashing equipment, chemical sanitization-temperature, ph, concentration, hardness (p,pf)
Inspector observed sanibucket at 0 ppm QAC. COS-Replaced with sanitizer per manufacturer's instructions (200-400 ppm). Corrective Action: A quaternary ammonium compound solution shall have a concentration as indicated by the manufacturer’s use directions included in the labeling.
16B - plumbing installed; proper backflow devices
Regulation: 511-6-1.06(2)(r) - system maintained in good repair (p, c)
Observed leak at the front hand sink in the main kitchen. Corrective Action: A plumbing system shall be maintained in good repair.
14C - single-use/single-service articles: properly stored, used
Regulation: 511-6-1.05(6)(r) - single-service/single-use articles, use limitations (c)
Inspector observed plastic mayonnaise containers being re-used. COS-discarded. Corrective Action: Single-service and single-use articles may not be reused.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(f) - drying mops (c)
Inspector observed mop in mop sink with mop head lying on the bottom of the mop sink. Corrective Action: After use, mops shall be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies.
